At Radfield Home Care Worcester, Droitwich & Malvern, person centred care has always been central to how we support people in their own homes. We are now introducing Life Stories, an NHS-supported initiative that helps people record and share the moments, experiences and values that matter most to them. By incorporating Life Stories into our assessments and ongoing support, we are further enhancing the personalised, compassionate home care we provide across Worcester, Droitwich and Malvern.
Taking the time to explore each individual’s experiences, values and preferences allows our Care Professionals to deliver care that reflects who someone truly is, not just the practical support they need.
Life Stories is a Herefordshire and Worcestershire NHS-supported initiative and online resource that helps people record meaningful moments from their lives. This can include family history, work experiences, hobbies, daily routines, beliefs and personal preferences.
By sharing this information, individuals can maintain their sense of identity, support emotional wellbeing and ensure care is delivered in a way that is respectful, familiar and deeply personal. For those receiving care at home in Worcester, Droitwich or Malvern, Life Stories adds an extra layer of understanding, helping our Care Professionals keep people connected to the things that matter most to them.
This approach fits perfectly with our Be There movement, as it enables our team to go beyond tasks, truly connecting with each person we support and focusing on what brings them comfort, joy and meaning in daily life.
We were pleased to welcome Tanya from Age UK Wyvern to our Radfield Home Care Worcester, Droiteich & Malvern office to introduce our team to the Life Stories resource and explain how it can be embedded into local home care services.
Age UK Wyvern is a respected local charity supporting older people across Worcestershire and Herefordshire. Their services include information and advice, wellbeing support, opportunities to stay socially connected and practical help to live independently with dignity and choice.
Their commitment to person centred support closely reflects our own approach to home care in Worcester, Droitwich and Malvern, and we are grateful for their guidance as we introduce Life Stories into our care delivery.

Life Stories provides a meaningful way for families to be involved in their loved one’s care journey. Contributing to a Life Story can help spark conversations, strengthen relationships and preserve memories that may otherwise be lost.
For families supporting someone living with dementia, memory loss or complex health needs, Life Stories can offer reassurance that care at home is being delivered in a way that truly reflects the person their loved one has always been.
While Life Stories is available online, we recognise that not everyone feels confident using digital tools. To ensure this initiative is inclusive, Radfield Home Care Worcester, Droitwich and Malvern is working with Age UK Wyvern to access paper based Life Stories resources.
These can be completed with support from our Care Professionals and family members, ensuring every client receiving home care in our local community has the opportunity to share their story in a way that feels comfortable and empowering.
